Three Brothers Lose Battle Against COVID19

One virus killed three brothers- the COVID19 pandemic took away the lives of three previously healthy brothers living in Tehran, Iran

Three brothers, who were previously healthy and had no history of any chronic medical condition including diabetes mellitus, hypertension, cardiac-, liver-, or renal diseases, lost their battles against the unforgiving COVID19 infection. They were in the age range of 54 to 66 years and lived in Tehran, Iran.

Brother (X), the first of the three to contract the infection, was 60-year-old and worked in a travel agency. He developed a fever but did not warrant hospital admission. After fever, the next day he developed a dry cough followed by dyspnoea 1 day later. Therefore, the doctors admitted him to manage progressive dyspnoea. Moreover, since the pandemic was in full swing, the doctors tested him for COVID19. His SARS-CoV-2 reverse transcriptase-polymerase chain reaction (PCR) test was positive. However, his spouse tested negative. Since the dyspnoea was worsening, he warranted an intensive care unit (ICU) admission. Unfortunately, 3 days later he succumbed to death due to respiratory failure.

The second to be infected, brother (Y), met his brother X one week before brother X developed his symptoms. Brother Y was 54 years old and worked in the same travel agency. He developed fever 2 days after brother X developed symptoms. Since he had a contact history, the doctors immediately admitted him to the hospital. A day later he developed cough and dyspnea. The doctors started the national treatment protocol for COVID-19 (hydroxychloroquine plus oseltamivir).

By the 5th day of admission, his condition improved.

Therefore, the doctors discharged him from the hospital with an O2 saturation of 87%. Despite adequate care at home, his condition deteriorated, and he died on the 9th day of his illness. Although his SARS-CoV-2 reverse transcriptase–PCR test was negative, the doctors strongly suspected COVID-19 infection.

Brother Z, the eldest brother, met brother X only for an hour on the 2nd day of brother X’s illness. Brother Z developed fever and cough 1 week later and the doctors admitted him with a strong suspicion of COVID19 given the history and the presentation. His dyspnea worsened in one day warranting ICU admission. Unfortunately, he also lost his battle against COVID19 and died secondary to acute respiratory distress syndrome. His SARS-CoV-2 test was positive; however, his spouse and their two children tested negative.
