Postmenopausal woman presented with giant ovarian serous cyst adenoma.

A 57-year-old postmenopausal woman was referred to the hospital from a local health centre with a 6-month history of a palpable pelvic mass. She had been menopausal for 7 years and gave birth to her last child 17 years ago. Her medical history revealed that she had hypothyroidism and was on Thyroxin 50 mg once daily. She had no significant surgical history.

The patient was asymptomatic and had no complaints of anorexia, nausea, vomiting, weight loss or postmenopausal bleeding, in addition to normal bowel and bladder habits. General examination showed that the patient had an average built with 62 kg weight. On abdominal examination, a pelvic mass was palpable, extending beyond the umbilicus. On palpation, the mass was mobile, firm and nontender. Vaginal examination showed that the cervix was normal with obliterated fornixes because of presence of mass.

Laboratory tests were unremarkable with tumour markers within normal limits. Radiological examination showed a normal sized and shaped uterus. A large left adnexal cyst was evident. The cyst was bilocular, thin smooth walled and had clear anechoic contents. It measured around 17.5 x 17.3 x 9.5 cm.

After normalisation of thyroid hormones, total abdominal hysterectomy with bilateral salpingoophorectomy was performed because of her age and menopausal status. Histopathological analysis was suggestive of benign serous cystadenoma of the ovary. The patient’s postoperative period was unremarkable. Ambulation and oral feeding was started 12 hours after surgery. The patient was discharged on the 4th postoperative day.
